Futures+ is a specialist Adult social care team that works across the whole of the East Riding.
We are seeking an enthusiastic and motivated qualified social worker to join us in supporting adults and young people with a Learning Disability and or Autism to maximise their independence and quality of life.
In addition, the team currently offer support from the ages of 14+yrs, and support people to make the transition into Adult Services.
As the successful candidate you will be responsible for assessing and support planning for people and their carers to ensuring that they have choice and control over their future under the Care Act 2014.
You will need to be prepared to undertake BIA training or be a qualified BIA assessor. You will need to have an excellent understanding of the MCA, Best Interest processes, safeguarding and Community Dols.
The successful candidate will be a registered social worker with post registration experience of at least two years in an Adult Care Management setting or similar. You will need to have an excellent understanding of the needs of customers with a learning disability, and or with Autism and evidence complex case management. It is essential that the post holder understand the needs of working within a multi-agency arena and have excellent working knowledge of The Care Act, The Children and Families Act, Mental Capacity Act and the Preparing for Adulthood agenda.
You maybe required to supervise un-qualified staff within the team. You will also have a proven track record of being able to use your own initiative in managing complex cases in a multi-agency. It is essential that the successful candidate has excellent communication skills and works in a person centred approach to their own case load.
