We are looking to recruit a band 6 OT to work within our existing CAT team based in Cannock covering the South West district.The core remit of the service is to provide Rehabilitation, Reablement and Recovery to those individuals who have been discharged from hospital with the emphasis of getting them back to their previous level of independence.You will be working closely with qualified and non-qualified therapist’s team with the remit of supporting individuals who have been recently discharged from hospital back home with Home first reablement support or to a rehab bed in a local care home. Your role will be to maximise individuals independence and reduce the need for long term services.Midlands Partnership NHS Foundation Trust is an award winning organisation with over 9000 employees.
