The Community Support Team provides services to people living in the community and covers the whole of the East Riding.

We are seeking to recruit a keen and enthusiastic individual to join our team of Assessment Officers on a 37 hours per week contract, working over a 7-day rota.

The Assessment Officer role is to support customers in the community by; completing initial assessments, risk assessing, undertaking medication assessments, and reviewing customers’ packages of care to determine any long-term care needs or appropriately signposting them for alternative services.

The Community Assessment Officer position is essential to the running of the Community Support Services. Our focus is reablement, supporting people to regain as much independence as possible following an illness/hospital stay. We are also a provider of last resort for customers in crisis, along with providing essential support for unpaid carers.

The position is crucial to the running of the Community Support service and is integral to meeting the current service demands and service changes.

The candidate will:

  • Have experience of working in a social care or another care related field, along with staff rota management (this is essential)
  • Have a good standard of education, written and verbal communication skills and an adequate level of literacy and numeracy
  • Be competent and confident in using IT packages, software, and equipment
  • Be able to set realistic goals for customers
  • Be able to effectively communicate and liaise with staff and other professionals at all levels
  • Be able to drive and have access to a car when at work
