Community Nursing Sister/Charge Nurse – Berkshire West

The Community Nursing Team in West Berkshire (Newbury) is an integral part of our Integrated Community Services.

The team has a wide range of skill mix from band 3 Health care assistants through to band 8a Advanced Nurse Practitioners. We have worked hard to establish a service that offers a range of career development opportunities for our staff which enables us to deliver continuity of care, Job satisfaction and high standards of care.

An exciting opportunity to join our innovative and successful community nursing team has arisen.

The post holder will be part of an area Community Nursing team and will have a leading role in the long-term condition management of patients, provision of admission avoidance services as well as contributing to the training and development of colleagues.

The Post Holder Will
  • Be highly motivated and assist the Integrated Community Services Team Leaders to develop and deliver high quality clinical services.
  • Develop strong working relationships with colleagues in general practice, social care and across BHFT.
  • Work across boundaries with the wider stakeholders to deliver best possible care to our service users.
  • Have a high level of clinical knowledge and clinical reasoning skills.
