Hope House Children’s Hospices have an opportunity for a Transition Lead Practitioner to join our team, based in either the hospice in Oswestry or Conwy. You will join us on a full-time, permanent basis working office hours. In return, our Transition Lead Practitioner will receive a competitive salary of £37,466 - £43,449 per annum. Which is the equivalent salary to top of Band 6 and top of Band 7 within the NHS.

About The Transition Lead Practitioner Role

We are seeking a dynamic and enthusiastic individual who can take the lead on the operational delivery of the Transition Service. You will work alongside the wider teams at Hope House and Tŷ Gobaith, and external care providers.

Your focus will be on developing and delivering the service in an empowering and inclusive way, at all times ensuring that Young People up to the age of 18 years of age, and their families, have their own voice as transition is made into the adult care system.

As an experienced practitioner, you will demonstrate an ability to put young people with life-limiting conditions, and their families at the very heart of your work. You will need to assess, plan and co-ordinate the delivery of age appropriate services to support identified needs, including Advance Care Planning.

This role will appeal to an experienced Children’s Social Worker, Social Care and Health Practitioner or an experienced Nurse. This interesting and varied role will provide you with an opportunity to progress and develop your career within a friendly and nurturing Hospice ethos and environment.

Demonstrates effective leadership skills

Has the ability to multi task and possesses excellent communication skills

Is flexible and adaptable in your approach to work and demonstrate effective decision-making skills throughout
