We have an exciting opportunity for a dynamic and motivated Qualified Therapist to Lead our Home First / Reablement service for Stroud locality.

As the Home First /Reablement lead you will be part of the leadership team for Stroud Locality alongside your county colleagues working autonomously and strategically.

You will be an experienced manager with skills to lead change and forge strong relationships within the Acute Trust and across the healthcare community. Both a flexible can-do attitude and a strong focus on delivery are essential.

As the Service Manager you will have responsibility for leading and managing the Home First /Reablement service for Stroud Locality.

You will ensure there is robust and safe clinical, and financial management of the Service Line and work with the Service Directors to ensure that the strategic direction, as defined within the service, is effectively implemented at an operational level.

You will lead on projects across the service in conjunction with leadership team leads for Stroud Locality Integrated community teams and Senior Managers. With this in mind, you will need to have a strong operational background with a proven track record of delivery of results, working with clinical teams, able to think innovatively and have the ability to influence, motivate and inspire others. Having a clinical background will be an added advantage.

You will have proven experience in budget and staff management with the ability to develop continuing relationships with Key Partners.

Gloucestershire Health & Care Services deliver community health & social care services for adults & older people in line with Primary care networks. Home First reablement teams working closely alongside their Integrated community health colleagues and health care communities, to provide support to people within their own homes and within bed based Reablement units. We work closely with GP’s, local communities, acute and community hospital(s) and other partners supporting a health coaching approach to enable people to live as independently and for as long as possible within their own home.

The post holder will work as part of the ICT managerial leadership team delivering the day to day business of the Home First and Reablement service and supporting its ongoing development as a key part of a successful health and care system.

While the post holders will have their own clear lines of responsibility they will work as a single cohesive management team. They will recognise and draw from colleague’s wider skill set within each team in delivering, together the localities day to day operational delivery and agreed developments. This will require the team deliver and model to multi-disciplinary working practices.
