This post is for a General Practitioner to supply primary care to a marginalised group of patients who have difficulty in accessing primary care through normal routes.
The successful applicant will supply primary medical care to several groups of patients: Specialist autism in-patients, Forensic learning disability in-patients and forensic mental health in-patients.
Inpatient stays at Northgate Hospital can be for lengthy periods of time and patients often have difficulty accessing primary care due to geography (they are away from their home area) and risk (the patient’s presentation may mean that accessing a GP surgery is not possible); therefore they require GP input to manage acute and chronic health issues normally managed within primary care.
The successful applicant will work alongside a triage/physical health care nurse who serves as the primary interface with the wards. There will also be opportunities to work with psychiatry colleagues and the wider MDT for patients when dealing with particularly complex or challenging issues.
The successful applicant will hold surgeries which patients will be booked into and also will be involved in carrying out annual health checks. In addition there will be a need to review some patients in situ on a ward if there is a particular clinical issue which prevents them attending a surgery. There will be clear opportunities to discuss patient problems as part of a multidisciplinary review if the management of the physical health issue is interacting with the patient’s other problems.
