We at the Northumberland Older Adults Community Treatment Team are looking to recruit a caring and compassionate Community Practitioner to join our team based out of Hexham covering the whole of Tynedale area and into fringes of Newcastle boundaries . We are keen to receive applications from individuals who hold a professional qualification in the field of health and social care; and have mental health experience.

We offer assessment and treatment to clients experiencing moderate to severe mental health difficulties including cognitive, physical co -morbidity or other age related complexity, using the stepped model of care, whilst fostering a multi-disciplinary approach.

The role requires an element of autonomous working and leadership skills, and you must be able to demonstrate a strong commitment to the delivery of high quality, safe and effective care and treatment.

You will work in a highly supported environment as part of the wider Older persons Pathway in which we offer opportunities to access additional training to further your development, and to be involved in future service development.
