Job content
The post holder will be responsible for the delivery of clinical care to a defined population. Supported by senior practitioners, they will deliver care within the boundaries of their role.They will work effectively and collaboratively within the district nursing team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership.
Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.

To assess, plan, develop, implement and evaluate programmes and treatment plans to promote and prevent adverse effects on health and wellbeing.
Prioritise health problems and intervene appropriately to assist patients in complex or emergency situations
Promote and deliver evidenced based care
Assist senior practitioners as required
Utilise and demonstrate sensitive communication styles - ensure patients are fully informed - recognise alternative methods of communication to overcome differing levels of understanding, cultural backgrounds and preferred ways on communication
Assess effectiveness of care through benchmarking, peer review and formal evaluation
Participate in maintenance of governance systems and processes across the organisation
Provide accurate contemporaneous patient records
Recognise and work within own competence and professional code of conduct
Evaluate the patients response to health care provision and effectiveness of care
