Our client in the Lincolnshire area is currently looking for a number of Newly Qualified Social Workers/ Care managers to join their Adults and older people’s teams across the county.

There is Positions available in Skegness, Market Rasen, Louth and Boston. The CCO role will cover Older People and Physical Disabilities in Adult Care. Primarily undertaking Care Act Assessments/Reviews. These will be 6 months assignments.

There is some flexibility with the office workers use based on location. Successful candidates won’t be expected to be in the offices all the time. There is availability for a mix of office and home-based working. Visits will need to be undertaken in the area they have been assigned to. We may also need to move areas in line with demand for assessments

You Must Have Experience Of The Following
  • Constructing care plans and packages to meet the needs of service users and carers
  • Purchasing services to meet assessed needs, seeking the appropriate authorisation from the budget holder
  • To carry out needs led assessments of adult within the guidelines of Community Care Act
  • Co-ordinating and reviewing multi-service care packages
  • Working in partnership with service users and carers using a person-centred approach, with the aim of maximising independence, well-being and choice
  • Attending and participating in meetings regarding service users, e.g. case conferences, network meetings, and reviews
What We Require From You
  • Suitable candidates must be a qualified social worker (DipSw, CSw etc..)
  • All appointments are subject to CRB clearances
