Children’s Community Nurse based at Mayfield Special School Term Time only. Non Term Time Supporting in the Children’s Community Nursing Team Torbay.This role is to provide nursing support, advice and interventions within the multi-agency ethos of Children and Family health Devon, for children and their families. Provide case management of an identified caseload of children with complex and/or long-term conditions by utilising and promoting evidence-based care.You will assist in promoting the health and well-being of children with complex nursing needs, to enable them to achieve the maximum benefit from their educational opportunities by supporting them to express their views and ensuring that their opinions are considered.
  • To promote and lead on providing a strength/asset based approach to the service delivery.
  • To develop effective patient centred decision-making processes.
  • To promote ways in which services can project a more positive and socially inclusive view of health and social care.
  • To engage fully across all areas of business focusing on improving the physical and mental health of all serving populations.
  • To ensure a culture where children, young people and their families/carers and the public are involved in the decision-making processes of services and in the wider context of the Alliance.
  • To promote patient and public involvement in all areas of responsibility
  • To develop strong relationships with partner agencies to provide an effective co-ordination of care for children young people and families.
  • To ensure that children and young people get heard, get support and get on.
