At Leicester City Council, we are changing the way we deliver adult social care and support services. We work closely with Health colleagues in the Integrated Locality Teams to maintain the health and wellbeing of people in Leicester as a shared responsibility. We work within a wider context of individual’s own abilities, family and wider community support in a strength-based approach. The model of care is based upon best practice, preventative care, promotion of self-care and independence and facilitates supporting named and accountable GPs in their role by ensuring that care and support for the patient is coordinated - especially across primary care, social and voluntary services but also between community and secondary care services.

This is a fantastic opportunity to join our friendly and welcoming team as a Care Navigator. You’ll work with colleagues to ensure each individual has access to support and physical resources which can reduce risk, avoid hospital admission and promote independence, including elements such as a personalised care plan, opportunities to access self-management training and support to be safe in their own environment. You’ll co-ordinate care effectively to ensure the right services reach the right people at the right time based upon a dynamic assessment of their needs.

The two position are full-time permanent roles, one based at Beaumont Way Social Care and one at Merlyn Vaz HSCC, however, you’ll be required to travel across the city to visit people in their home and to attend meetings. You’ll be provided with a laptop and a mobile phone so that you are able to work flexibly. On occasions you may be required to work out of normal office hours, depending on the needs of the service.

Your role

As a Care Navigator, you’ll act as an essential link between Primary Care, Community Health Services, Secondary Care, Mental Health and Adult Social Care by contributing to and, where appropriate, coordinating the planning of care. You’ll ensure that each service involved in the care plan is aware of the individual’s requirements and has scheduled in delivery of their care by ensuring links between the individual, General Practitioner (GP), community health services and adult social care are established and maintained. You’ll provide advice and support on lifestyle changes to individuals by referring them on to appropriate services, e.g. stop smoking, continence service, dietary services etc as well as support multi-disciplined care teams within each locality in line with needs and demands. You’ll also be a Trusted assessor to arrange aids, equipment’s, minor adaptations and AT, carry out home assessment, warm home and benefit advice and access to housing support in LA. On request from a GP you’ll also carry out minor health observation checks, BP, temp, oxygen sats, wheelchair referrals, etc.

What you’ll need

You’ll have experience of co-ordinating a number of care services to individuals in terms of planning and delivery as well as experience of providing support and advice to individuals and their carers. With experience of partnership and multi-disciplinary working and using your excellent interpersonal skills, you’ll be able to effectively communicate at all levels to establish and maintain good working relationships with individuals, their carers and a range of professional colleagues and stakeholders. Using your proficient IT skills and good attention to detail, you’ll be able to keep accurate and timely records as well as monitor, observe and record details of specific outcomes and health outputs. With good organisation skills, the ability to work under pressure and a flexible approach, you’ll be able to prioritise tasks.
