Community Link Worker (FTC, Part-Time)
Covering four GP practices across Dunbar and East Linton

We are currently looking for a Community Link Worker to join us on a fixed term contract until the end of March 2024.

Overview

Royal Voluntary Service are a well-known and respected charity which have been working across East Lothian for over 35 years, with services currently funded by East Lothian Health & Social Care Partnership.

The Role

As a Community Link Worker, you will use a social prescribing approach to support people and identify issues that affect their ability to live well.

Based across the GP practices throughout Dunbar and East Linton, you will help people to identify personal outcomes and priorities they would like to address. You will aid them in overcoming any barriers and link them to local and national support services and activities.

You will begin by conducting 1-1 assessments which will involve a structured conversation allowing the person to open up and speak about the various barriers and issues they might be experiencing in their lives. You will also ensure adherence to safeguarding policies and health and safety guidance.

Additionally, you will:

  • Support the existing GP practice teams
  • Ensure a positive uptake of the service by linking in with key partners
  • Compile a directory of organisations which can support the delivery of the Community Link Worker Service

Working Hours

This is a part-time role, working 21 hours per week across three days, on a fixed term contract until 31st March 2024.

About You

To join us as a Community Link Worker, you’ll need:

  • Experience of working in partnership with local and statutory organisations
  • Experience of service delivery co-ordination or project management level work
  • Caseload management experience
  • Good IT skills, including proficiency in MS Word and Excel
  • Experience of working within community learning and development, social work, health or another related field would be an advantage

A full, valid driving licence and access to your own vehicle would be beneficial to your application, as would experience working in a primary care setting.
