Who are we?
Rapid Response and Intermediate Care (RRIC) is a team of Physiotherapists, Occupational Therapists, Intermediate Care Nurses, Community Healthcare Assistant Practitioners and Healthcare Assistants. The Aylesbury team is one of 7 teams across Buckinghamshire and is based at Stoke Mandeville Hospital.
What do we do?
The team provides specialist assessment and rehabilitation to patients in their own home to empower and enable their recovery from illness or injury and maximise their independence. We support people to prevent unnecessary hospital admissions and facilitate timely discharges from hospital as well as provide care to those at the end of life. Our clinicians provide holistic comprehensive assessments, expert clinical interventions and set up appropriate rehabilitation plans for support staff to follow. We pride ourselves on the high quality, compassionate care our team provides across the county.
We promote an integrated approach and work with colleagues in primary care, community nursing, hospital and social care to support people in their own homes.
What does the role involve?
Providing expert assessment and intervention for patients at home. Managing a caseload of complex patients and supporting the team to manage the wider caseload. Actively work with other teams and agencies to proactively manage patients’ needs. Providing professional and operational leadership for the local team and supporting the RRIC Manager. Supporting staff to develop their knowledge and skills.

About You

You should have
A broad range of clinical expertise, with extensive experience of working in a community multidisciplinary team environment within the NHS. Management skills and experience of leading a team An innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery. A full driving licence and access to a car for work purposes.

Additional information
In order to respond to the healthcare needs of the local population, our community teams operate 7 days a week ensuring the delivery of a service which can react to patients’ needs as and when required. The shifts for RRIC Advanced Community Practitioners are between the hours of 08:00-20:00 ( 7.5 hour shifts ) including weekend and bank holiday working as part of a rota. Currently RRIC Case Managers work approximately one weekend per month and one weekday late shift (12:00 -20:00) per week. Weekend and Bank Holiday working is subject to enhanced pay in accordance with Agenda for Change Terms and Conditions.
