We have a fantastic opportunity for a Band 8a Quality and Development Lead for the West Berkshire Urgent Community Response Team.You will provide advanced clinical assessment skills to patients in their own home and in a care home, who require an urgent response to prevent unnecessary admission to hospital. The post holder will work as part of a multi-disciplinary team to provide appropriate patient focussed care and clinical leadership to the team.The role will provide day to day management of the West Berkshire UCR multidisciplinary team, including nurses, therapists and support workers, and clinical supervision and training/ education to the nurses and support workers, as required.You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.The post holder will work 37.5hrs per week, 7.5 hours a day, Monday to Friday between the hours of 8am – 8pm.Interviews to be held 12th October 2022To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service in collaboration with the Community Geriatrician and GP.As an experienced advanced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.You will be expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate mannerAs an Independent Prescriber, prescribe and review medications in accordance with NMC guidance, local and national policy guidance.To provide leadership and line management to the Band 8/7/6 nurses and therapists and the Band 3 & 4 healthcare/multi-therapy assistants.Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC, information governance and local / national guidance.The 3/4 “must haves” for you to be considered for this role:
  • Advanced Practitioner registration or working towards obtaining registration
  • Advanced History taking and assessment
  • Non-medical prescriber
  • Able to work autonomously, with the ability to work under pressure, with excellent organisational, communication and time management skills to prioritise work
