Job Overview

At Wirral Community Health and Care NHS Foundation Trust, we pride ourselves in providing the best support to members of our local community to maintain their independence and dignity to help them live as well as possible, for as long as possible. We are looking for proactive and dynamic individuals to join our Integrated Care Coordination teams in an Integrated Community Health and Social Care service, working across the Wirral Peninsula.

You will work with people aged 18+ who may have multiple conditions, learning disabilities and complex care needs as part of a multi disciplinary team in a community setting.

To join our Integrated Care Coordination teams you will:

  • Be a registered Social Worker with Social Work England
  • Have completed ASYE and critical reflection modules post qualification
  • Have proven knowledge of the Care Act 2014, Mental Capacity act 2005, Safeguarding and other specific legislation appropriate to Adult Social care
  • Be committed to delivering a high standard of care and promoting professional standards
  • Be an excellent communicator who will innovate, inspire and support colleagues
  • Advocate for members of our community, identifying changes in need and planning for their current and future care

An opportunity has arisen to work within an integrated dynamic, innovative and friendly team within Wirral Community NHS Foundation Trust’s Access and Intermediate Care Division

An opportunity has arisen for a dynamic and motivated, band 5 Social worker to join the service, which also involves a role in the multi- disciplinary team within the Discharge to Assess bed-based service and Integrated Discharge Team.

We are looking for an enthusiastic and forward thinking social worker to join our busy, pro-active, integrated team based within various across the Wirral.

The role will involve working with people with a wide variety of both physical and mental health conditions as well as complex social issues. This role will involve collaborative working and taking responsibility within our experienced multi-disciplinary teams- MDTs, to ensure best possible outcomes are achieved for people using the service, their carers and families. You will attend regular MDT meetings and ensure that you work effectively to provide person- centred care, ensure timely journey for people who use the service, prevent discharge delays and avoidable admissions to hospital and long term care.

You need to have social work experience, have an excellent understanding of current social work legislative practice. There will be a need ensure that you work in compliance of the requirements of The Care Act 2014.

Risk management, excellent communication skills and an ability to prioritise and often work at a fast pace is essential. You will need good time management skills.

This post requires applicants to have completed their ASYE. It is also essential that the post holder is able to effectively resilient to challenges posed by the system during winter pressures.

The post holder will be required to actively participate in quality improvement initiatives and to ensure that they manage their continuous professional development. There will be the opportunity for regular supervision and to attend in-service training and professional development. The team has an active social work focus group, to which membership will be offered

The service operates with extended hours 7 days per week providing cover 8am-8pm. You would be expected to work to cover service demands across the extended hours and weekends on a roster basis.
