We are currently recruiting Social Care Practioner for our Hospital team part of Physical Disability and Older Persons Team. You will be working to promote independence, support with direct payments, protect vulnerable adults and working with Health, voluntary and independent sectors.

You will come to the role with energy, experience and a good understanding of Adult social care. In return we will provide you with the tools to do your job, excellent supervision and development opportunities to progress your career in Social Work.

Successful applicants will be skilled at communicating and working with people with social care needs and with people with a variety of physical and cognitive impairments. You will need to have excellent communication skills and the ability to work professionally in a multidisciplinary/multi-agency context. You will need to have competent IT skills and be able and willing to learn to use a variety of software systems.

To facilitate as required the implementation of interventions in order to meet residents identified outcomes and achieve their optimum functioning and independence using 3 conversation model (EST)
