The Adult Social Care Locality Team are offering an exciting opportunity for a Social Care Officer post. This role would be supporting island residents to achieve independence in their own homes/community by taking a strengths-based, person centred approach to promote independence and positive wellbeing, enabling the client to achieve their goals and aspirations. This will involve undertaking Care Act Assessments, reviews, and creative care plans, while working in partnership with service providers and qualified Social Workers.

You will have good communication and relationship building skills and fully recognise the importance of ensuring that the voice of those we are supporting is heard. You will share our ambitions, passion and attitude to ensure the people we serve live the best lives possible.

The successful candidate will be enthusiastic, motivated and passionate about working with a range of clients over the age of 18 and be suitably qualified with a NVQ3 in Care or equivalent. You will be able to demonstrate knowledge and understanding of The Care Act, Mental Capacity Act, Data Protection Act and evidence of ability to work to practice guidance/procedure. You will be required to take part in the Out of Hours (OOH) stand-by duty rota as and when requested by management on an occasional basis.
