The National Spinal Injuries Centre (NSIC) provides lifelong care to people who have sustained a life-changing Spinal Cord Injury (SCI). This role incorporates 2 key areas in the patient pathway ranging from the point of referral to the point of discharge.

The Post Holder Will Oversee The Following Teams

Acute outreach team who liaise with patients and professionals whilst the patient is awaiting transfer to the NSIC.

Case Management team who are fundamental to the facilitation of safe discharge from hospital in liaison with the patient and their local services.

The post-holder will provide management and leadership to Acute Outreach and Case Management within the NSIC and will be pivotal in the management of patient flow, ensuring effective communication with key stakeholders to ensure that patients receive the best care throughout their journey.

A key member of the multi-disciplinary team, this individual must be passionate about safe care provision to this patient cohort.
