This is an exciting role to coordinate and develop our Transitions Wellbeing work across the Wakefield district.

The transitions work in Wakefield encompasses two strands. Firstly, supporting young people that have previously engaged in child and adolescent mental health services (CAMHS) and are now over 16 years old and need further support in order to help them live an independent, healthier life. This support is based on the young person’s needs and whilst typically focuses around mental health, emotional and wellbeing support, can also extend to variety of areas such as housing, financial, career and social support.

Secondly, providing an enhanced community-based outreach support service to young people aged 12-19 who are identified as SEND and facing mental health difficulties impacting their ability to engage with educational provision. This group may include young people with a mental health diagnosis, those on waiting lists with lower level support needs, those living in difficult family situations, which all impacts their ability to engage or attend educational provision or positive activity.

This offers a unique opportunity to work with a young person for a minimum of 5 months to identify their needs, work with them to develop an appropriate support package and help them in their journey to recovery. This holistic approach means the work can be extremely varied and lead to engaging with the individual across a number of settings and with a number of different agencies and organisations to best support that young person.

Wherever possible for sustainability and support, we are keen to make community links and partnerships, the successful applicant will have the exciting role of building exciting new relationships in Wakefield and have the opportunity to map and build our knowledge of community provision and support as well as developing our signposting, community engagement and onward referral links.

This unique role will have a case load of young people as well as time to supervise a small team and build a new transitions team across Wakefield. We currently have expertise in Transitions work delivering across Leeds, therefore this role will be supported and expected to work alongside our Senior Transitions Worker in Leeds. As this is such a varied role, there will be ongoing opportunities for learning, training and personal development to ensure the successful applicant feels confident and competent to support the clients and the team across a variety of areas.
