This is an open-ended position required for 37 hours per week. Core hours Monday - Friday but may be requested to work over 7 days.

Pembrokeshire County Council are engaged in an exciting transformation of their Intermediate Care Services. As part of this, we are looking to appoint new staff for our Reablement Team as we look to expand and develop this service.

The successful candidates will work as part of the Integrated Intermediate Care Service in Pembrokeshire which supports people who require a multidisciplinary approach to recover and maintain their health, wellbeing and independence. The Intermediate Care Service includes responding to people at risk of admission to acute or residential care, supporting discharge home and delivering therapy led Reablement.

You will be central to delivering a short term, outcome based service that maximises and maintains independence for our service users. You will be assessing individuals, devising goal/ outcome orientated care plans, monitoring and reviewing progress and delivery. For this, you will work closely with the Reablement support workers, involved in developing their skills and competencies for direct interventions.

We are looking for a creative, motivated, and focussed individual who has experience of working in a multi-disciplinary setting. Understanding of relevant legislation, policy and the underlying ethos of Reablement and Intermediate Care is essential. You will need to have a relevant professional OT qualification.

Local Government Employees on National Joint Council (NJC) terms are subject to national pay bargaining; all salaries currently stated in our adverts are as 01/04/2021 as the pay award from 01/04/2022 has yet to be finalised.
