As our Lead Support Worker, you’ll be at the heart of supporting our customers who have complex mental health needs to develop their skills to live more independently in the community. Typical day as a Lead Support Worker (known by us as Coordinator)

  • You’ll be part of a team of 6 Support Coordinators who will be super passionate about supporting customers to reach their goals and celebrating their achievements. You will report into a Client Service Manager.
  • Creating support plans with your customers and coordinating their discharge from hospital with planned support in the community to maintain their tenancy, wellbeing and mental health to prevent re-admission. Our small team of Support Workers working to the plan.
  • You will support our brilliant customers who have mental health and housing support needs. Our customers could live in a community-based setting in their own homes or are inpatients in our local Mental Health Hospitals and rehabilitation wards.
  • Helping customers with lots of activities from budgeting to maintaining their tenancy and moving onto independent or supported accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
  • To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

Job details

  • Flexible working hours
  • Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
  • Working Monday – Friday 9am – 5pm and managing your own diary and time.
  • Although you’ll lone work, we do get together regularly for training and team meetings.
  • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  • You’ll need an Enhanced DBS check done and we pay for that.

Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external candidates for this role.  This remains under regular review.
