Job Summary

Health Care Assistant - Band 3

Sapphire ward is looking to expand. The service is unique and differs from the rest of the inpatient services across the Brooklands Site. The vast majority of the assessment and treatment needs of the patients are completed for this patient group . We specialise in complex and often prolonged transitions of patients with complex needs that either cannot live in a main ward environment or require slower, longer transitions in a lower arousal space.

We are looking for enthusiastic staff to join the team on sapphire as we expand the service into phase 2. We require committed, reliable, motivated, compassionate, outgoing and energetic individuals to join us in developing our service further. Individuals should be person centred in approach, willing and able to support patients with complex needs. They should be able to work under direct supervision and be willing and able to work independently in guiding and sharing knowledge with external transition staff. This is a key component of the role of the frontline staff on the ward.

The ethos of the ward is to prepare patients and bridge the gap through transition from inpatient to community. Individuals must be willing and able to support patients to accessing community facing facilities. Individuals must be skilled in supporting and promoting independence by supporting patients to participate and maximise independence in activities of daily living skills on and off the ward.

As a Health care Assistant, You will be required to assist patients with Activities of Daily living, personal care, support leave on site and in the community and to complete observations. You will be required to work closely with the Qualified Nurses and all members of the Multi Disciplinary Team so that assessment and treatment can be completed in a timely manner. You will be assigned as a member of a care team for an allocated individual and will play a very important part in the patient’s transition back into the community.

As an expanding service we are looking for individuals that are confident in supporting the Operational manager, clinical lead and wider multi disciplinary team into continually developing an improving the service through involvement in care planning, PBS implementation and improving patient care and experience from those that use the service and families/carers.

This is achieved over a 24 hour period on a rota system and includes days, nights, weekends, bank holidays (including Christmas and New Year)

Whilst full induction and training will be given due to specialist nature of the service it is crucial that staff have a particular interest in this group. The post holder will be required to provide emergency to other units in event of temporary staff deficits.

Qualifications

Person Specification Qualifications Essential

  • Hold NVQ Level II in Health and Social Care and be willing to work towards NVQ III, willing to undertake work-based assessments
  • Willing to undertake the Care Certificate (Learning Disabilities)

Experience Essential

  • Experience of working with people with a learning disability.
