This is an exciting 0.5 WTE Band 8a clinical specialist physiotherapist post, supporting a 24-month project for people with kidney disease who are medically fit to enter the kidney transplant list but require to lose weight. Working closely with a clinical specialist dietitian and clinical psychologist, this post will continue to develop, implement and evaluate this innovative service in its second year. The post originated following a successful grant request to the NHS England . The successful candidate will hold a clinical caseload which will be closely integrated with the MDT service development. As a clinical specialist physiotherapist you will act independently, leading and supporting this project through the data collection, plus communication with Renal and Therapies leadership team and work by following the 4 pillars of advanced practice.

Main duties of the job

You will be responsible for the development, implementation, monitoring and evaluation of a new service delivery for people who have kidney disease, medically fit to enter the transplant list but need to lose weight. You will be responsible for a number of patients who will be regularly followed up over the course of 24 months. The role is flexible with some home working and face to face duties. You will work as part of the a dynamic and proactive MDT. Experience in exercise management of complex patients, including patients with kidneys disease with weight loss considerations is essential. Additionally you will have responsibility for providing regular updates to the project lead, the renal MDT and therapy team. The project will be based at the City Campus and our satellites unit. You will be responsible in ensuring the delivery of this including looking at different ways of working and thinking innovatively.

As an autonomous clinical specialist physiotherapist, you will be responsible for ensuring safe practice through development of a caseload management plan and to identify treatment priorities in a variety of situations. You will provide expertise and input into the strategic planning and implementation of multidisciplinary clinical guidelines, care pathways and protocols within the trust, in line with national and regional guidance within specialist areas of practice.

To provide clinical leadership within the specialist service, through consultation and joint working with all bands of staff, in the management of complex patients, this will involve review of their clinical reasoning, physiotherapy assessment and treatment plans.

To maintain and develop high standards of patients care and facilitate and implement evidence based practice by ensuring implementation of standards set in relevant national guidance, specialist clinical group guidance and specialist relevant areas. Also by ensuring staff are informed of recent developments through attendance at research based conferences, relevant network meetings and postgraduate education and training courses.

To develop local pathways of care for specialist area patients

To evaluate the effectiveness of the service through service evaluation & clinical audit in liaison with the Therapy Services Manager and relevant stakeholders.

Where appropriate, offer extended scopes of practice so as to facilitate the optimal management of patients within a clinical setting.

To ensure waiting list targets are maintained by initiating and developing new ways of working (e.g. telephone consultation, ‘opt-in’ appointments, SOS appointments).

To ensure high standards of clinical care on issues pertaining to specialist area, to own team and a wide range of disciplines (e.g. medical staff and the MDT) in line with the education and training strategy by:

To establish collaborative links with other local and national advanced practitioners and disseminate information and development.

To develop links between local physiotherapists to enable the sharing of best practice (e.g. benchmarking, Essence of Care).
