To lead on the liaison relationship between Rise CYP Mental Health services and Primary Care. Your role will be to support clinical decisions and discussions for CYP who are not known to Rise services or those who are open to Rise Mental health care so as to support GPs who need advice. You will lead in the approach to review CYP referred from primary care and assess where there needs can be best met. Your role will involve offering professional advice, joint support, and advice to aid discussions on CYP needs in primary care who have been discharged or not engaged with Rise services, ensuring that the needs of the CYP are navigated to the right place alongside the GP.The role will provide an opportunity to support primary care and increase access for clinically evidence based interventions for mild to moderate MH needs of CYP. The roles will enhance CYP journeys and create better joint working across primary care and CYP mental health care services.To offer brief interventions to CYP who have mild to moderate mental health needs in their community. The role will include delivering low intensity Evidence based interventions with some short term case management. You will in your role co lead on the youth worker role as part of the PCN offer as well as work closely with social prescribers and other professionals within the PCNs multi-disciplinary teams, to support the CYP to access the right services in the right place and at the right time.To provide support to the locality PCNs as part of a team to ensure patients are seen and provided support/interventions/therapies based on needs chairing the referral meeting and ensuring relevant actions are coordinated to support plans of care for CYP.The post holder will be a resource of clinical knowledge and competence in the service area. The post holder will be expected to supervise other staff.Provide dedicated support and presence within primary care settings Build long-term relationships and improve communications with primary care colleagues and other stakeholders.For more information on this role please see the attached support information.
