Our client is a family-run Domiciliary Care company covering the Wiltshire area. They specialise in care and daily living assistance to a wide range of clients who require support due to aging, illness, recovery, or rehabilitation. The Role They are looking for a competent and experienced Care Co-ordinator to work as part of a small friendly team in their busy head office in Lacock. Due to the nature of the business, it is essential that you have a full clean driving licence and your own car, as you may be expected to visit clients on a daily basis.Additionally, they would prefer that you live within otheir areas of business to be able to get to emergencies quickly and assist in cases of sickness. These areas are currently Calne, Chippenham, Melksham, Corsham, Devizes and surrounding villages. Your duties will include:-Planning day to day rotas for carers, re-planning work to cover sickness or emergencies.
  • Writing and updating care plans and maintaining client information on our digital software systems.
  • Liaising with clients, their families and other related contacts as required in order to support the needs of our service users.
  • Domiciliary care work as and when required by the business; this may include evenings, weekends, bank holidays, Christmas and New Year.
  • Covering the "on-call" phone service - this duty is currently one evening per week and also weekends on a rota basis with other office staff (days off in lieu are given).
  • Other required tasks on an ad-hoc basis in support of the business, our services users and your colleagues. Essential skills/qualifications of a Care Co-ordinator:
  • NVQ Level 3/Diploma in Health & Social Care.
  • Experience in planning staff rotas, ideally using CarePlanner or a similar system
  • Experience in writing and maintaining client care plans, ideally using PASS or a similar system.
  • Experience in hands-on care support work.
  • Experience in working in a busy office environment, with proven skills in the use of MS Office packages
  • Confidence in dealing with contacts face to face and on the telephone with a professional yet personal approach.
  • Keen attention to detail and accuracy.
  • Ability to prioritise own workload and escalate where necessary.
  • Strong teamwork ethic.
  • Customer-focused, with excellent listening skills and a high standard of verbal and written communication. Desirable skills:
  • Experience in the use of CarePlanner.
