Milton Keynes University Hospital are looking for a enthusiastic Assistant Practitioner to lead a group of Therapy led Health Care Assistants across a number of inpatient wards supporting a 6 month ‘Keeping You Active’ programme.

The aim of the keeping you active programme is to promote and empower patients to maintain and optimise independence by encouraging them to participate in activities of daily living, based on their own preferences, choices and habits. Reducing the risk of deconditioning associated with a hospital stay and improving patient flow into the community.

You will be responsible for attending ward board rounds to coordinate and oversee patients identified by the registered therapist (Occupational Therapist or Physiotherapist) or nurse to participate in the keeping you active programme. You will work closely with the Therapy HCAs to support with:
  • On-going training needs to ensure that patients are reaching their functional goals.
  • monitoring patient progress to identify individuals that require additional therapy review and treatment goals for discharge.
  • clinically assessing and progressing/adapting treatment programmes of appropriate patients as delegated by a registered therapist.

  • To monitor stock of equipment and ensure stock levels are maintained in the therapies department.
  • To provide supervision as required to Therapy Assistants and students.
  • You will also be responsible for promoting and educating the Multi-Disciplinary Team (MDT) on the importance of keeping patients active during their hospital stay and supporting with the collection of quality improvement data to demonstrate patient outcomes.

  • To undertake assessment, treatment and discharge of appropriate patients as delegated by registered physiotherapist (PT) or Occupational Therapist (OT), adhering to agreed departmental guidelines & protocols.
  • To organise and run exercise programmes (group or individual), assess and provide appropriate mobility aids, measure and fit adaptive equipment, promote functional independence through transfer and mobility practice sessions.
  • To be responsible for maintaining own competency to practice through CPD activities and to maintain a portfolio which reflects personal development.
  • To participate in the staff appraisal scheme as an appraise and be responsible for complying with your agreed personal development programmes to meet set knowledge and competencies.
  • Liaison with and teaching of therapeutic skills to other health care professionals, students and agencies e.g. social services.
  • To be responsible for planning own caseload on a daily basis to meet service and patient priorities, readjusting plans as situations arise and change.
