We have an exciting opportunity for a Mental Health Support Worker to join our inspiring and friendly Sheffield Outreach team. Your role will be a fixed term position until 3rd September 2021, working full time, 35 hours per week.
Our Station Grove service is a supported housing service with 11 self-contained flats and communal areas in Southport town centre. We provide medium to low supported housing for individuals with a mental health diagnosis aged 18 and over requiring tenancy, mental health, emotional and living skills support.

What will you be doing?
- Providing key worker support to individuals living with mental illness, including one to one individual or group support, liaising with other mental health agencies on behalf of people who use our services
- Enabling individuals to self-manage their personal and domestic resources and to undertake activities of daily living
- Enabling and encouraging individuals to find out about local services and facilities within their community and facilitate access to alternative services where specific needs are identified
- Familiarising myself with individual Care/Recovery Plans and support people who use our services in achieving their goals
- Carrying out all need’s assessments, individual safety and support planning within organisational and legislative requirements.
- Contributing to the protection of individuals from the risk of abuse and harm to self and others who use our services
- Increasing choice and involvement of service users and carers
- Maintaining accurate records as required by policies and procedures, using appropriate paper and electronic systems. To maintain up to date details of service and support activities and outcomes on the Rethink Information System
- Experience of providing support to people with a mental illness; or experience of providing care and/or support services within another social care setting in either a paid or voluntary capacity.
- Knowledge and understanding of recovery principles and embedding the ethos of recovery into your own practice.
- The ability to make a comprehensive assessment of an individual’s situation and needs; to develop, implement and review risk management plans and devise support plans to meet these needs.
- QCF Diploma or NVQ level 3 in Care or Certificate in Community Mental Healthcare (or willing to work towards) or an equivalent qualification.
