Job Summary

  • The post holder will be part of the Urgent Community Response Team that provides a seven-day service for rapid assessment, and intervention for adults of all ages who do not require specialist acute hospital admission, but who require crisis intervention within a community setting. This may be due to an alteration in their physical or psychological well-being, or a temporary change in their social circumstances which makes it difficult for them to be maintained in a community setting without a short period of rehabilitation or intervention from the service.
  • The post holder will provide office-based support for the Urgent Community Response /Virtual ward Team and support the community divisional administration team.
  • Co-ordinate and prioritise referrals into Urgent care response/virtual ward from both acute and primary care.
  • Work proactively within the Urgent community Response /virtual ward team to support within an extremely busy office environment.
  • Work flexibly to meet the needs of the service.
  • The post holder will be a key member of the urgent community response /virtual team and will contribute to the delivery of a quality service and maintenance of quality standards.
  • The post holder will need to be an excellent communicator, motivated and adaptable to the changing caseload.

Main duties of the job

  • To co-ordinate Referrals Received within the Urgent Community Response/virtual ward Team.
  • To communicate effectively and in a timely manner, with relevant clinical staff and other colleagues: to ensure referrals are actioned within the appropriate time frame.
  • Maintain and distribute documents, including reports, spreadsheets, and data basis.

Job Description Job Responsibilities

  • Participate in service audits where appropriate.
  • Order and receive clinical supplies, including stock taking where necessary.
  • Ability to work autonomously, plan, prioritise and organise own workload, whilst meeting specific deadlines of the team.
  • Maintain strict confidentiality in conjunction with Data Protection act.
  • To be flexible to changes in service needs and undertake training as and when necessary.
  • To carry out any administrative functions as requested including referral to other services.
  • Able to fulfil the health requirements of the post as identified in the Job Description, taking into account any reasonable adjustments recommended by Occupational Health.

Qualifications

Person Specification QUALIFICATIONS / TRAINING Essential

  • GCSE English and Maths
  • NVQ level 3 or equivalent in appropriate area - office admin
  • Good standard of general education
  • Commitment to ongoing training and development
