This is an exhilarating opportunity to apply for this amazing, awe-inspiring and thrilling position of Transition Nurse Specialist at Dorset County Hospital Foundation Trust.

We are seeing a motivated, engaging and energetic nurse to support young people, and their families, transition successfully to adult services. You will be the lynchpin in making a difference, improving patient care and patient experience, celebrating the positive impacts achieved and sharing learning widely.

The successful applicant will empower and support young people with a variety of conditions to transition from children’s to adults’ services gradually, positively and without fear. This is a golden opportunity to make a distinct difference to young people who await your passion and care. You will be working with an amazing close circle of fantastic support and resources within Children’s Community Services and wider Paediatric Team. We place young people and their families at the heart of the service and we take pride in our ability to support each other in meeting the goals of the team.

We are committed to the ongoing expansion of our service to address the growing health needs of young people and their transition needs. The successful applicant will also join a transition steering group at Dorset County Hospital and a wider regional transition network.

Are you that motivated and energising clinical professional, with knowledge and experience in transition, excellent interpersonal and communication skills, with the ability to organise and plan effectively and a good all-round knowledge of IT skills?
