We have an opportunity for a Therapy Practitioner in our Community Therapy South Team based in Worthing, so if you’re an ambitious and proactive individual looking to work in a team that provides excellent therapeutic care, we would love to hear from you.

This is an amazing opportunity to become a member of the Community Therapy Team providing proactive community rehabilitation and urgent care pathways to help patients stay in their homes, avoid unnecessary hospital admission and support them to live as independently as possible.

Our teams work flexibly to provide a seven-day service, 365 days a year, between 8am and 8pm. In this role, your day will be varied and interesting, managing clinical risk within your own patient case load, and supporting patients towards achieving more independence in their activities of daily living. You will be responsible for prescribing, and competent use and maintenance of equipment relevant to your service area.

You will complete non-complex MDT assessments under the direction of a registered health professional and be actively involved in MDT meetings. You will implement patient -centred treatment programmes and will regularly monitor, evaluate, and modify these programmes, with clear documentation. Promoting self-management and providing education, advice and signposting will be key to delivering the best for our patients.

You will be part of our supportive team, ensuring that you feel confident and able to undertake this role to the best of your ability, whilst being able to reach your own potential through personal development along with peer and supervisory support.

You will need to able to drive with a full UK licence due to the geographic area covered by the team.

Key responsibilities
1. To be clinically responsible for a caseload of patients who have been delegated by a registered member of staff.
2. To organise your caseload on a day to day basis effectively and efficiently with regard to priorities and use of time.
3. To review and reflect on your own practice through regular participation in clinical supervision and annual Personal Development Review (PDR) with nominated supervisor, as per Trust policy.
4. To support the registered clinician with the clinical supervision of Band 3 staff.
5. To provide clinical observation opportunities for undergraduate and work experience students.
6. To participate in training programmes, staff meetings, audit and clinical governance.
7. To demonstrate appropriate duty of care in relation to equipment and resources.
8. To be responsible for maintaining and developing own competency to practice in line with agreed Personal Development Plan (PDP), through Continuous Professional Development (CPD) activities and maintain a portfolio which reflects personal development.
9. To participate in continuing professional development activities individually and as part of the service.
10. To undertake relevant activities to meet training objectives in line with the Calderdale framework.
11. To participate as necessary in service development, service audit and outcome measurement to ensure the provision of evidence-based practice.
12. Gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
13. With appropriate training, demonstrate an enhanced level of specialist knowledge linked to the delegated patient caseload of the team and to be able to work with adults/ children with a variety of conditions.
