£46,836 - £52,849 Inc HCAS per annum pro rataThis is a wonderful opportunity to join a well-respected, established multi-disciplinary respiratory team.The successful candidate will work within the wider consultant led integrated respiratory team across the area to provide seamless care for patients with chronic lung disease. The team delivers a seven day admission avoidance /supported discharge service alongside optimising patients referred from GP’s , which may include attending pulmonary rehabilitation which the team also provides.The successful candidate will also have the opportunity to become involved in the prescribing and monitoring of domiciliary oxygen. The physiotherapists in the team also offer advice and education on chest clearance techniques if requiredTo work as a highly specialist member of the respiratory team. This team provides expert knowledge and advice on the management of local patients with various respiratory conditions. The team works closely with other teams and secondary care to provide a fluid patient pathway.The post holder will therefore maintain a high standard of patient focused care reflecting an interdisciplinary approach to rehabilitation and self-management.The post holder will support and develop junior members of the team to prioritise, assess, treat and manage a caseload of patients, as well as managing their own caseload of patients as an autonomous practitioner, including the maintenance of comprehensive records.The management of highly complex patients in the community will require monitoring using telehealth and linking closely with the Acute Trust.To work as a highly specialist nurse/ physiotherapist within Bromley healthcare Respiratory Service to manage clients referred to the Multi--Disciplinary Team-including complex patientsTo assess patients for supplemental oxygen and then monitor patients on home oxygen to ensure a safe and cost effective provision as per guidelines.
