The Stay Well at Home team (SW@HT) are looking for their next clinical specialist Occupational Therapist. The successful candidate will join an established and award winning team focused on preventing unnecessary admissions ’at the front door’ within Accident and Emergency and medical assessment units.

We are looking for an enthusiastic and highly skilled individual who is able to work within an integrated health and social care team. You will work with a team of Occupational Therapists, social workers, physiotherapy and assistant practitioners. An ability to undertake swift but thorough assessments of individual functional performance and develop a plan to overcome potential challenges to discharge is essential. In addition you will be responsible for identifying and prescribing packages of care.

Your role will also involve identifying and contributing to areas of service improvement and ensuring good governance is maintained within teh team through regular audit, education, training of staff members.
