This is an exciting opportunity for an individual who is interested in palliative and end of life care to join our dynamic specialist team, with the bonus of retaining existing NHS pension scheme membership if you have one.

We are seeking an enthusiastic and highly motivated Registered Nurse to join our multi-disciplinary team in the provision of specialist palliative care to patients and their families at home.

It’s a role where you’ll have the luxury of precious time to devote to individual patients and make a real difference to their quality of life when it really matters.

Our team works within a Clinical Hub structure. Reporting to the hub clinical lead, this role will work alongside a multidisciplinary team of nurse specialists, healthcare assistants, social care and Allied Healthcare professionals.

The role of the Clinical Nurse Specialist is to manage patients with complex holistic needs, clinically assess and plan care to meet a patient’s practical, psychological, spiritual, and social needs.

You will work closely with primary and secondary care professionals and hospice colleagues based at our inpatient unit and in hospital. Good communication and team working skills are essential.
