Oxford Health’s Buckinghamshire Mental Health Directorate is working in collaboration with Buckinghamshire’s Primary Care Networks (PCN) to develop and implement improved access to specialist mental health care and treatment within primary care services. Focusing on bridging the gap between primary and secondary care Mental Health services for individuals with complex and severe mental illness.

The service is designed to empower patients 18years+ to access specialist mental health support locally, within their GP Surgeries in a timely manner.

We are looking for autonomous, driven, and confident Social Work(s) who pride themselves on delivering a high-quality holistic approach for patients needing mental health support. The successful Social Worker will facilitate improved opportunities for joint working with our voluntary care sector, contributing to the leadership and development of national mental health agendas within their Primary Care Network (PCN). As part of the Primary Care Networks Multi-Disciplinary Team (MDT) your role will provide mental health triage, assessment and advice to patients, carers, and colleagues. Facilitating early interventions to aid recovery within the community and utilising your profession specific skills to enable prompt access to specialist services for patients.

As the Social Worker in this post, you will be expected to use your profession specific skills and values in this role and apply Social Work theories to practice, such as rights and strengths based approach, alongside your expert knowledge of the Care Act eligibility and safeguarding principles to support your clinical practice and signposting to appropriate services.
