Specialist Doctor in Elderly Medicine10PA with elderly medicine oncall.Harrogate and District NHS Foundation Trust (HDFT) are looking to appoint to a Specialist Grade role as part of an expansion in our frailty services. This post will have a commitment to out of hours working for the elderly virtual ward and acute frailty unit. The post is for 10PA, which includes a 1 in 10-weekend working for the acute frailty team.The development of frailty services is a priority for the Trust, both in the acute site and in community services. We are looking for an enthusiastic colleague to join our frailty ward team and acute frailty team. The successful candidate would be integral to providing a robust service for our most frail patients in the hospital. This is an exciting time to be joining our expanding service, with a new acute frailty admissions ward; we are able to streamline the care of our patients to ensure they receive the right care in the right location. Our new hospital at home service allow us to deliver care across the community, working closely with our dynamic ARCHES team which has been very successful at enabling our ‘home first’ ethos, and admission avoidance schemes.You will be joining a dynamic and enthusiastic team of 12 Consultant geriatricians with complementary skills, and with special interests in acute care, community rehabilitation, movement disorders, stroke medicine, orthogeriatrics, continence, dementia, falls and polypharmacy. We are also supported by three specialist advanced clinical practitioners and 4 registrars, as well as a diverse and highly valued multidisciplinary team, who share our passion for delivering high quality person-centred care. As a community and acute frailty geriatrician, you will also work closely with colleagues in acute and general medicine, and in the Emergency Department. We are a provider of undergraduate medical school education for the University of Leeds and have a thriving research community.
