Our Client is providing care and support to individuals who just left hospitals and need support while on rehabilitation process in own homes.
This is more Senior Role and will involve independent working and ongoing contact with the office to report and update any changes in rehabilitant’s health.

Duties Of The Specialist Care Assistant
  • Reviewing and updating individuals care plans and risk assessments on a daily and weekly basis
  • Assisting with any client’s needs on a daily basis
  • Promoting independence and Person Centered Care
  • Participation and assistance with any needs assessments and Completing weekly reviews of outcomes
  • Supporting individuals with personal care and day to day tasks
  • Signposting clients to services which will ensure their continued independence after the rehabilitation programme service has ceased
Requirement Of The Specialist Care Assistant
  • Experience in care setting
  • Flexibility in working hours and ability to work alternative weekends
  • Good communication and reporting skills
  • Ability to work independently and as a part of the team
  • Strong attention to detail
