We currently have vacancies for a qualified Social Worker to join our team in the Discharge Hub at York District Hospital or in the Hambleton South Community Social Care Team based at Thirsk Highways, to help us realise our ambition of ensuring that all residents of North Yorkshire live longer, healthier, independent lives. This unique opportunity in the Discharge Hub is a full time, established post. Based in York our team predominantly covers hospital discharges emanating from York District hospital sites including York, Selby and Easingwold. We also support Mid Yorkshire hospitals, Pinderfields, Dewsbury and Pontefract and other outlying hospitals where people being discharged from hospital live in the Ryedale, Selby, and Hambleton areas of North Yorkshire. Working in a fast-paced environment, you will work closely with our health colleagues to ensure that seamless pathways of care are delivered under Discharge to Assess arrangements. You will undertake assessments under Discharge to Assess guidelines whilst being mindful of the importance of a strengths-based focus and preventative measures to ensure we have the person’s wellbeing at the heart of what we do. Mental capacity assessments and best interest decision making and s117 assessment and care planning of people being discharged from hospital and safeguarding vulnerable adults will be part of your day to day work. We can be creative with the approaches that we use to ensuring that people have their needs met in a safe manner, whilst clearly noting the importance of thinking ‘Home First’.

As a Social Worker in the Hambleton South Community Social Care team, you will undertake a range of asset and strength based assessments and interventions to ensure that adults with care and support needs and their carers achieve their desired outcomes and wellbeing to live independent lives. You will be responsible for developing personalised care and support plans that represent best value, mental capacity assessments and best interest decisions, determining eligibility for adults with care and support needs against the national standards and ensure that they are safe from harm but where harm or abuse is identified ensure the safety of the person and undertake the relevant safeguarding enquiries.

To be eligible for this post you will have a professional Social Work Qualification, DipSW or CQSW and you will have current registration with Social Work England.
