This is an exciting opportunity to be involved in our Integrated Care System which is bringing organisations together to wrap care around the individuals we support.

The Integrated Care Team in RBWM is a dynamic multi-disciplinary and multi-organisational team that came into being just over a year ago, and through embracing the Integrated Care model, has had great success with supporting our health and care system and the residents of RBWM.

We have built solid relationships with Primary Care, Community Health and Social care and have become the “go-to” team to support individuals with multiple service needs and challenging presentations.

This role would suit a Social Worker who has the client at the heart of care planning and who enjoys building relationships, and working with, learning from, and being supported by a wide group of professionals in a service that is evolving and developing with the changing system.
