Beacon Medical Group is looking to recruit a Social Prescriber to provide support for our patients within our Primary Care Network.

The person appointed will be responsible for directing patients to services across the health sector, voluntary sector and other statutory agencies who can assist with their ongoing needs. To take a holistic approach to addressing non-medical/social issues in their life, such has housing, debt or social isolation by connecting people to community groups, activities or services.

Main duties of the job

The successful candidate will need to have a comprehensive knowledge of activities/services available across Plymouth and South Hams and be able to demonstrate enthusiasm and the necessary interpersonal skills to become an effective support to our patients.

We are looking for a motivated individual who is able to work independently to manage their own workload and are flexible to respond to the developing nature of the role and the needs of individuals requiring support.

We strive to provide consultations for patients so they receive the Right Care, with the Right Person, at the Right Time, including a paramedic led Urgent Care Team, making the best and most appropriate use of every clinician’s skills. Care is provided by practice nurses, clinical pharmacists, mental health nurses, social prescribers, general practitioners, patient advisers, healthcare assistants, paramedics and phlebotomists, and we contribute to the education and professional development of trainee nurses, medical students and GP Registrars.
