The Amber Valley Older Adult Community Mental Health Team (CMHT) is looking for a new Service Manager/Dementia Lead. This is a busy team supporting service users with both functional and organic illness.

The role of Service Manager / Dementia Lead involves managing all aspects of the daily functioning of the team. The team is multi-disciplinary in nature, comprising of Nurses, NMP, OT’s, Support Workers and Psychology, working closely with the dedicated Consultant.

A passion and interest in working in the area of older adults is essential. The role involves working closely with our partners in care within the PCN’s, Social Care and the local VCSE.

The Older Peoples Division is a supportive and actively encourages the development of leadership skills. Robust supervision will be given, support from peers and access to training and development.
  • Day to day operational management including supervision, caseload management, appraisals, referrals and allocation.
  • Management of sickness/absence.
  • Performance management and compliance with KPI’s.
  • Collaboratively working with the ASM and peers in development and service improvement plans, implementing these within the team.
  • Recruitment and selection.
  • Leading with the teams Care Homes Project.
  • Developing relationships with partners in care, including GP’s, Social Care and the VCSE.
  • Leading clinical and operational team meetings.
  • Investigations, dealing with complaints, concerns and the management of risk.
  • Ensuring the team adheres to policy and procedures.
  • Budget management and CIP.
  • HR concerns.
