The Community Care Team is at the forefront of delivering NHS England’s ambition of “boosting out of hospital” care and developing and testing new models that provide support to patients closer to home.

As set out in the 22/23 NHS planning guidance, the transformation of out-of-hospital services is a key element of the NHS recovery. Systems have been asked to increase overall capacity of community services to provide care for more patients at home and address waiting lists, develop and expand new models of community care and support timely hospital discharge.

The Community Care Team is recruiting to several roles across its Strategy and Policy and Service Policy, Design and Implementation functions, to deliver the Community Care portfolio across England. This is an excellent opportunity to contribute to a major high-profile national programme and make a difference to specific population groups, such as those with multi-morbidities, in England.

The Community Care portfolio currently includes:
  • development of Community Care strategy and policy, particularly working closely with primary care, social care and urgent and emergency care colleagues and partner organisations
  • designing new models of Community Care in partnership with other parts of the health and care system (e.g. Anticipatory Care Operating Model development)
  • driving roll out and effective implementation of new models of care (e.g. Anticipatory Care and Urgent Community Response)
  • working to understand the impact of Community Care models and enable quality improvement (e.g. delivery of the Enhanced Health in Care Homes Framework)
  • developing and delivering the transforming children’s community services programme, with a particular focus on reducing waiting lists and provision of community care services for children with special educational needs and disabilities).
