There has been significant investment in additional posts to support the transformation and Hull City Council are looking for exceptional individuals like you to join the Adult Social Care team.

You will work as part of an integrated multi-disciplinary team to provide high quality social work practice; ensuring individuals have a solution focussed plan towards achieving their goals and maintaining their independence. You will work creatively with individuals, local communities and our partners, empowering them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture.

We are looking for individuals who can demonstrate:

  • A strong value base that informs your work with adults

  • A capacity for honest reflection and openness to learning;
  • Ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers

You must be a qualified social worker and be registered with Social Work England.
This post will provide and support expert and effective social work practice in complex situations, assessing and managing high levels of risk and providing expert opinion in relation to decisions surrounding complex cases. You will undertake a professional leadership role taking responsibility for the learning and development of others, through leadership and management, supervision, mentoring, assessing and teaching.

The Locality teams support all individuals requiring longer term adult social care support (excluding individuals who are supported by Humber Trust for specialist Learning Disability or Mental Health input). Referrals to the team will come directly from See & Solve Community, for ongoing support following Active Recovery and after a hospital discharge package has been put in place. The team work jointly, between Social Work and Occupational Therapy as well as with Sensory and Telecare colleagues, to identify the goals an individual wants to achieve and the best way to achieve these. The team also ensure individuals receive timely and proportionate reviews ensuring that an individual’s goals and actions are reflecting changes in their life and support them to be independent.
