We are currently recruiting Senior Social Worker within our Physical Disability and Older Persons Team. The Senior Social worker role will be part of Hospital Social Work team. The Hospital team is based in Town Hall, Maidenhead and works collaboratively with the hospitals and multidisciplinary professionals from hospital to facilitate safe discharges for the residents.

The Hospital team operates Monday to Friday 08:45am-5: 15 pm and on pro rota basis working on Saturday.

You will come to the role with energy, experience and a good understanding of Adult social care. In return we will provide you with the tools to do your job, excellent supervision and development opportunities to progress your career.

Successful applicants will be skilled at communicating and working with people with social care needs and with people with a variety of physical and cognitive impairments. You will need to have excellent communication skills and the ability to work professionally in a multidisciplinary / multi-agency context. You will need to have competent IT skills and be able and willing to learn to use a variety of software systems.

To facilitate as required the implementation of interventions in order to meet residents identified outcomes and achieve their optimum functioning and independence using 3 conversation model.
