NELFT provide mental health services for Children and Young People across Kent and Medway. We are now able to offer an exciting opportunity for a Children’s Senior Social Worker to join the newly established Inpatient, Enhanced Treatment and Crisis Pathway which offers three distinct pathways.
  • Inpatient Pathway– based in Staplehurst, comprises of 14 beds for young people who require admission to support them in managing their mental health. The site also houses the Health Based Place of Safety (HBPoS). The purpose-built unit is the main base for the whole pathway and includes a broad and experienced Multi-Disciplinary Team (MDT) which includes Doctors, Psychologists, Occupational and Creative Therapists.
  • Intensive Home Treatment team (IHT) – with the main hub based at the inpatient unit in Staplehurst, IHT provides community based enhanced and intensive support to young people as an alternative to admission. There is blended and flexible approach, with staff in IHT able to in reach into the inpatient unit or work with young people in the community at a number of locations. IHT has a broad range of clinicians including nurses, support workers and members of the MDT and offers interventions from 72hrs up to 12 weeks. IHT delivers the Kent and Medway Dialectical Behavioural Therapy programme and can support staff to access training and skills in working with the model. Staff work agilely across Kent and Medway to deliver interventions to young people.
  • Crisis Assessment Team (CAT) – with a focus on crisis assessments in the community and in A&E or acute hospitals, the CAT team are based agilely and mobilise to conduct assessments with young people and their families who are in crisis.
You will be supported by a dedicated and supportive leadership team and have the opportunity to participate in research, clinical modelling, and service development. You will also work collaboratively with the young people and their families and carers, other NELFT professionals, KCC, schools and other partner agencies to deliver outstanding care. There will be an emphasis on taking responsibility for communication and development of clear discharge plans for the young people within the described Tier 4 pathway. You will take a lead role in ensuring communication is clear within the network, with the voice of the young person at the centre.The team are operating a hybrid model of working across both office and agile community working.
