Job Summary

A unique and exciting opportunity has arisen for a Pharmacist to join our forward thinking Pharmacy team at Bury and Rochdale Care organisation and be at the forefront of developing clinical pharmacy services to the Planned and Integrated care divisions at Rochdale infirmary

Rochdale Care organisation has community based integrated care at its heart and they are leading the way in helping keep patients well in their own homes. Rochdale Infirmary is also a hub for day case elective surgical pathways within Greater Manchester.

This varied role therefore involves working as part of a multidisciplinary team with patients in all settings i.e. inpatients, intermediate care, day case or in their own homes, to provide high quality clinical pharmacy services

Whilst primarily based at Rochdale Care Organisation, you will work closely with other members of the pharmacy team providing pharmacy services across Bury and Rochdale Care Organisations

We are looking for a highly motivated, patient focused pharmacist who relishes the challenge of leading clinical pharmacy services to this cohort of patients

The post will provide the opportunity to develop clinical, leadership and managerial skills and utilise prescribing skills.

The post involves participation in the late night, weekend, bank holiday and emergency on-call rota for the Bury and Rochdale Pharmacy service.

Main duties of the job

You will work closely with clinicians and other members of the Planned and integrated care teams at Rochdale Care Organisation to deliver a high quality medicines management service as part of a dedicated, forward thinking pharmacy team.

The Main Responsibilities Will Include

  • Managing the day to day pharmaceutical activities of the pharmacists on rotation at Rochdale infirmary on the intermediate care units in medicines reconciliation, advance discharge planning, one stop dispensing, ward based dispensing and patient counselling. The role also involves working within in rapid response providing medication review in a domiciliary setting
  • Working alongside the multidisciplinary team in developing the services to planned and integrated care. You will be actively encouraged to contribute towards on-going service development and attend speciality level meetings.

Qualifications

Person Specification Qualifications Essential

  • MPharm Pharmacy degree
  • Pre-registration training and experience
  • Member of the General Pharmaceutical Council
  • Postgraduate qualification in clinical pharmacy

  • Non medical prescribing

Experience Essential

  • Extensive clinical experience within a hospital pharmacy

  • Audit design and service improvement

  • Therapeutics of common disease states and developments in medicine
