Nottingham CityCare Partnership is looking for Registered Nurses with community nursing experience, or a student nurse who has completed their management placement in the community, to join both the Integrated Care Homes Service.

The Integrated Care Homes Service comprises of health professionals who in reach into Care Homes within Nottingham City to deliver NHS services. The team comprises of community nurses, dementia outreach staff, specialist nurses, therapy staff and advocacy who work alongside each other to provide integrated holistic care. The service works in partnership with care home staff, pharmacists and GP’s. The aim of this innovative service is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs. The service provides training to care home staff and is involved with the implementation of innovative ways of working and the implementation of the Framework for Enhanced Health in Care Homes.

The successful candidates will require up to date clinical knowledge and skills and will be required to work autonomously, using initiative to deliver nursing care within a supportive team environment. They will be working with a diverse population providing support and nursing care to people who have complex health problems, with the aim to maintain and maximise their independence and prevent avoidable hospital admissions. You will be supported to learn and develop the skills required for this role.
