SERVICE AREA: ADULT SOCIAL CARE & COMMUNITY WELL-BEING POST TITLE: Re-ablement Leader, Ref: 50038796 TEAM NAME: Adult Social Care & Housing Needs - Regaining Independence Service, Salary: Grade 7, £20,983.92 to £22,268.51 per annum. Hours: 35 per week, Contract Type: Permanent It is a really exciting time to be joining us in Community Re-ablement & Outreach as we continue to expand our service to support more people with packages of care in their own home. Our service supports people upon their discharge from hospital as well as helping to avoid people being admitted to hospital. Following the internal promotion of one of our existing team we are seeking a Community Re-ablement Leader to join our team. You will be based within our community team in Sandown with travel expected island wide. Your role will be varied but rewarding. You will undertake assessments in peoples homes and you will manage a caseload of people we support, reviewing their care needs with them however your skills and experience will help you lead, develop, motivate and support your team of support workers. You will work closely with our Response Co-ordinators, colleagues in the Regaining Independence Team and Social Care Officers, supporting and facilitating peoples journey through the service. You will need to have experience of health and social care services along with knowledge of supporting people in their own homes with an understanding of completing care plans and risk assessments in a strengths based and person centred way. This is demanding roles that involves working a flexible roster pattern that includes weekends and bank holidays. You will be at the forefront of service delivery, leading by example and ensuring high quality services for people using our service and their families and carers.
