An exciting opportunity has arisen to join our Integrated community therapies service in the Swansea Bay University Health Board. This involves working in a variety of community based settings across Swansea Bay.

The post will be based in the Integrated north hub in Gorseinon hospital but may involve working across other sites of the service.

This post will provide an opportunity, as part of a multidisciplinary team , to provide rehabilitation to patients and groups of patients in a number of settings, including inpatients , patients own home and care homes.

The post holder will be required to support therapy input to the Home First pathway which supports people to return home from hospital as soon as they are well enough, allowing them to recover in their own home with or without care input.

The service is provided across the whole of the Swansea Bay area. Patients are also seen in the inpatient settings of Bonymaen house and Gorseinon ward as part of the pathway.

The post holder will need to be able to adopt a flexible approach to work and will also be expected to participate in 7 day working rotas.

You will need to be able to demonstrate significant experience of working within rehabilitation in NHS, Social Care or Community based settings.

Previous therapy assistant experience ,including community is highly desirable for this role.

An NVQ 3 and /or evidence that you are working towards the physiotherapy/occupational therapy support worker diploma or other relevant healthcare or exercise diploma is essential.

The ability to travel independently between sites across the service is also essential so the successful candidate must be able to do this in a timely manner.

The role will also entail some administrative responsibilities, so good basic IT knowledge and/or understanding of healthcare IT systems is essential .

You will be expected to provide support to the physiotherapy staff with patient programmes in the community setting.

You will be supported by senior therapists and the clinical team lead.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
