Job Summary

East community Rehabilitation Team are a small and dynamic therapy team providing community physio and occupational therapy to adults with physical health issues within the Easington Locality. The team also works collaboratively with the community nursing services working over a 6 day rota providing same day response for patients with nursing/ rehab and care needs that require a 2 hour crisis response. Domicillary experience and car driver essential. We provide cover for the east durham demographic.

As part of ongoing development within the team we are looking for an experienced dynamic Band 6 Physiotherapist to join us serving both the intermediate care and domicillary service.

You will have the opportunity to be involved with service development as part of the wider community therapy services and an opportunity to develop your clinical and leadership skills within the supportive environment of the east therapy team.

Main duties of the job

To work as a member of the clinical team of physiotherapists, occupational therapists and therapy assistants within community therapy services.

The post holder will independently manage a caseload that includes some highly complex patients and provide specialist and generic therapy assessments to adults referred to the team who present with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease, within a range of community settings and establishments (including Rehabilitation Wards/Units, Clients Homes, Care Homes and Prisons)

On an on-going basis the post holder will assess, diagnose, implement and evaluate individual programmes of care for patients.

The post holder will work within a multi-professional team (Consultants, GPs, nursing staff, Occupational Therapists, Social workers and other AHPs) regarding the management and discharge of patients under your care.

The post holder will deliver clinical education of all staff as required within community services

Job Description Job Responsibilities

Client Group: Adult rehabilitation to community based patient population over 5-7 day service as required.

To be an autonomous practitioner with the freedom to act to be personally, professionally and legally accountable for the services you provide to patients in your care and for those Associate Practitioners, Care and Therapy Support workers and

To perform advanced therapeutic assessment of patients with diverse presentations and sometimes highly complex physical conditions; to provide a diagnosis, develop, and deliver individualised treatment programme

To hold responsibility for own caseload and be responsible for a defined area of the service or a particular patient type, working without direct supervision.

To be operationally managed on a day to day basis by your designated clinical lead, and will receive formal supervision from a B7 therapist either within group or individual sessions.

To be supported by the designated Clinical Lead to carry out your duties via informal supervision and guidance on an as needs basis, dependant on your level of experience and complexity of patients under your care.

To work within established Service / Trust policies and procedures and contribute / propose changes for the development of operational procedures / working practices relevant to the team and services it provides.

To be responsible for providing formal clinical supervision and support to lesser experienced therapists, plus Assistant Practitioners and Care support workers. This includes personal and professional development, including appraisal as well as support for case management beyond their level of knowledge and experience. Informal support will also be given on a day to day basis to assist with caseload management.

To provide supervision, education and assessment of therapy students; this would be to a graduate standard and involve working with universities to ensure the standard of practice and teaching meets the standard set by the degree level qualification

To undertake evidence based audit to further own and teams clinical practice within the specialism. To make recommendations to the Clinical Lead of service that may lead to implementation of specific changes to practice or contribute to service protocols

Qualifications Essential

  • BSc Physiotherapy or equivalent
  • State Registration
  • Continuing Professional Development Diary including evidence of reflective practice, training and clinical experience

Desirable

  • Recognised relevant academic qualification
  • Member of CSP
  • An understanding of County Durham and Darlington NHS FT, its culture, strategic direction achievements and NHS standing

Experience Essential

  • Relevant experience in physical rehabilitation including recent general rehabilitation and community rehabilitation.
  • Evidence of NHS experience.
  • Experience in working in a variety of settings with evidence of generic transferrable assessment and treatment skills between physiotherapy and occupational therapy.
  • Ability to work within a multidisciplinary team
  • Evidence of communication skills within a wide variety of settings
